Initial 15 Minute Consultation – No Fee

Intake and Evaluation – $175 per session

Individual Therapy – $110 per 30 minute session

                                 – $140 per 45 minute session

                                 – $165 per 55 minute session

Couples Therapy – $165 per 55 minute Session

Consulting Discovery and Strategy Session – $250 per 60 minute session

Consulting – Fees varying based on project scope

Insurance

Depending on your current health insurance plan or employee benefit plan, it is possible for services to be covered in full or in part. Co-pays, deductibles and co-insurance payments are required on the date of service. All clients, insured and self-pay, are enrolled in autopay for same-day payment processing.

Clients with out-of-network insurance plans are required to pay for services on the date of service. Superbills (receipts of services) are provided for clients to file for reimbursement of covered services. The amount reimbursed is based upon each specific insurance plan. Please contact your insurance provider to verify how your plan compensates you for psychotherapy services.

It is recommended that you ask these questions of your insurance provider to help determine your benefits:

• Does my health insurance plan include mental/behavioral health benefits?
• Are tele-therapy services covered? If so, am I required to use a specific provider?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit the number of therapy sessions per calendar year? If so, what is the limit?
• Do I need a referral from my primary care provider prior to receiving therapy services.

Payment

The Crafted Path, LLC accepts all major credit/debit cards as forms of payment. Payment is required on the date of service.

Cancellation Policy

If you are unable to attend a scheduled session, please cancel at least 24 hours prior to your appointment. Otherwise, you may be charged a late cancellation/no show fee of $75.
